Instructions for completing the form

  1. Application document MUST be completed in its entirety.
  2. No more than four (4) persons can be included on an account.
  3. In keeping with Bank of Saint Lucia's AML policy, we require that ALL our existing customers' information remain up to date. As such the items listed in the Account Requirements attached must also be submitted by the existing customer(s) to support the updated information provided (In particular forms of identification and supporting documentation for any change of residential address).
  4. A Personal Information Form to be completed and signed by each signatory to the account.

Instructions for completing the form

  1. For A+ Club - This account must include at least one (1) minor.
  2. Others - For individuals 18 years & older ONLY.

  4. Savings application form-In a case where the minor cannot sign (below the age of 7yrs) the other party (parent/guardian) to the account becomes the primary account holder and the minor's information is placed in secondary account holder details.
  5. Only one (1) Savings Account Form - Minors should be completed and signed by all parties to the account.
    • Minors from the age of 7-17 years MUST sign on the savings account form.

What documents do you need?

Individual (National)

  1. One form of valid (unexpired) Government-Issued Picture Identification is adequate in the form of either:
    • Passport or
    • National Identification Card
    Driver's License is acceptable on condition that it is accompanied by one of the following:
    • Birth certificate
    • Other Government issued ID
    • Employee ID
    • School ID
  2. Confirmation of Residential Address document by way of:
    • Original utility bill in the client's name dated within the last 3 months
    • Tenancy agreement in the client's name
    • Reference Letter from a reputable Financial Institution stating permanent residential address (The relationship must be in existence for at least 2 years).
    • Letter from employer confirming address
    • For persons with combined living arrangements, a letter issued by the utility bill owner, along with copy of the ID of utility bill owner, and the original utility bill dated within the last 3 months
  3. Letter from your current employer (name, length of employment, position, annual salary) or other documented evidence of source of funding for account.

Individual (Non Nationals/Non Residents)

  • Items 2 and 3 in Individual (National) above, plus:
    1. Two Primary Forms of Government Issued Identification:
      • Passport and
      • Either Driver's License or National Identification Card
    2. One of the following issued within the past three months from a reputable Financial Institution:
      • Reference letter or
      • Account statement or online Bank Statement

Self-employed Individuals

  • Government-Issued Picture Identification and Confirmation of Residential Address under Individual National, plus:
    • Practicing Certificate/license (where applicable).
    • Letter providing a clear description of the customer's business activity (Required for Registered Business Accounts and optional for Savings Account).

Instructions for completing the form

  1. For individuals 18 years & older ONLY.
  2. Client(s) must already operate/maintain a savings account with BOSL for a minimum of 6 months.
  3. Credit assessment MUST be done for client(s) prior to approval and opening of account.

  5. Only one (1) copy of the Chequing Account Application should be completed & signed by all signatories to the account.

INDIVIDUAL ACCOUNTS

Individual (National)

  1. One form of valid (unexpired) Government-Issued Picture Identification is adequate in the form of either:
    • Passport or
    • National Identification Card
    Driver's License is acceptable on condition that it is accompanied by one of the following:
    • Birth certificate
    • Other Government issued ID
    • Employee ID
    • School ID
  2. Confirmation of Residential Address document by way of:
    • Original utility bill in the client's name dated within the last 3 months
    • Tenancy agreement in the client's name
    • Reference Letter from a reputable Financial Institution stating permanent residential address (The relationship must be in existence for at least 2 years).

Sole Proprietorship/Registered Business

  1. Certificate of Registration of Business Name
  2. Filed statement of particulars (Application for registration by an individual/firm)
  3. Full KYC requirements for the registered owners, Signatories, and individuals with Power of Attorney for registered owners and Signatories, where applicable. (Refer to Government-Issued Picture Identification and Confirmation of Residential Address under Individuals above)
  4. Letter providing a clear description of the customer's business activity (Required for Registered Business Accounts and optional for Savings Account).

What documents do you need?

  • Certificate of Registration of Business Name
  • Statement of Particulars
  • Two forms of identification of all beneficial owners and signatories. Photocopies must be notarized. (Driver's license, Passport, National ID or other state provided ID)
  • Original Bank Reference - owners of the account
  • Proof of Business address (Utility bill, Lease agreement, Statements etc.)
  • Audited Financial Statements for past three years/(Projected Cash Flow if new)
  • Introductory Letter
    (Indicating your interest to open accounts with us) This letter must also entail a brief overview of the business, list all signatories and state their capacity. Web address/brochure should be provided if any

NB: All references should be original and addressed to the Bank of St. Lucia, #1 Bridge Street, Castries, St. Lucia W.I. Financial references must be obtained from a reputable bank wherein the account holder has had a satisfactory banking relationship for a minimum of three years or from another reputable source such as an Attorney or Accountant verifying the applicant's satisfactory banking history.

All non-original documents must be notarized.

    • Tenancy agreement in the client's name. Where the agreement is more than 3 months old from the date presented, a recent receipt evidencing monthly rental payment must accompany the tenancy agreement. This application form ("The Application") must be used by Bank of Saint Lucia ("BOSL" or "The Bank") customers who are desirous of enrolling in the Bank's online and mobile banking ("e-banking") service ("The Services") as follows:

  1. Individuals (adults) eighteen (18) years of age and above. These individuals must have attained the age of eighteen (18) at the time of signing The Application prior to submitting it to BOSL for consideration.
  2. Individuals (minors) below eighteen (18) years provided that the parent(s)/ legal guardian(s) indicate consent in the AUTHORIZATIONS, AGREEMENTS, AND DECLARATIONS section of The Application and sign The Application accordingly.
  3. Registered businesses (Sole traders only).

General Information regarding this form:

  1. Your enrollment in the Services is dependent on BOSL's acceptance of the signed The Application and your acceptance of BOSL's electronic banking (e-banking) terms & conditions (The Agreement). For additional information, please obtain a copy of The Agreement at any BOSL branch or download a copy from the BOSL website at https://www.bankofsaintlucia.com/

Instructions for completing the form

This application form ("The Application") must be used by Bank of Saint Lucia ("BOSL" or "The Bank") customers who are desirous of enrolling in the Bank's online and mobile banking ("e-banking") service ("The Services") as follows:

  1. Businesses. These include but are not limited to: partnerships, companies, embassies, statutory bodies, groups, clubs, and associations and societies.

General Information regarding this form:

  1. The person to whom The Application refers as the desired user, must sign The Application.
  2. Please fill in this form to apply for BOSL's "The Services". Once you have completed all applicable sections of this form and signed accordingly, please submit The Application to the Bank as follows:
    • In person to your preferred BOSL branch, where the Bank will complete the application process while you are in branch.
    • Via the post: The Application must be duly notarized and then posted (postal service). Alternatively, you may submit the notarized form via a courier service or other third party.
    • Via email: The Application must be duly notarized and then e-mailed as a scanned attachment to bosldigital@bankofsaintlucia.com, using an e-mail address that you provided to the Bank previously.
  3. Your enrollment in the Services is dependent on BOSL's acceptance of the signed The Application and your acceptance of BOSL's electronic banking (e-banking) terms & conditions (The Agreement). For additional information, please obtain a copy of The Agreement at any BOSL branch or download a copy from the BOSL website at https://www.bankofsaintlucia.com/
  4. If BOSL accepts The Application, during the enrollment process, BOSL will provide you with a username and password (credentials) which will allow you to access your online and mobile banking account. Your credentials will be provided to you as follows:
    • In person, if you submitted The Application and complete the sign-up process at the Bank in person.
    • Via e-mail combined with a phone call from you, if you submitted The Application to the Bank via post or e-mail

    • Reference Letter from a reputable Financial Institution stating permanent residential address (The relationship must be in existence for at least 2 years).
    • Letter from employer confirming address
    • For persons with combined living arrangements, a letter issued by the utility bill owner, along with copy of the ID of utility bill owner, and the original utility bill dated within the last 3 months
  3. Letter from your current employer (name, length of employment, position, annual salary) or other documented evidence of source of funding for account.

Instructions for completing the form

  1. Application document MUST be completed in its entirety.
  2. No more than four (4) persons can be included on an account.
  3. In keeping with Bank of Saint Lucia's AML policy, we require that ALL our existing customers' information remain up to date. As such the items listed in the Account Requirements attached must also be submitted by the existing customer(s) to support the updated information provided (In particular forms of identification and supporting documentation for any change of residential address).
  4. A Personal Information Form to be completed and signed by each signatory to the account.
